New Student Registration
Only the legal parent/guardian can register a student. If your name is not on the student’s birth certificate, you must bring court documentation pertaining to custody of the child.
For Grades K-12:
STEP 1:
Completion of Final Forms

STEP 2:
Schedule a mandatory registration appointment. Access to this schedule is found on the first form within your new student’s final forms account, labeled “New Student Registration Process.”
Please review the required document checklist. Your child’s enrollment will not be finalized until all required documents have been submitted to the registrar.
- Child’s birth certificate
- Parent/guardian photo ID
- TWO proofs of residency: Lease agreement or purchase agreement AND current utility bill. These must be in the parent/guardian’s name.
** If you do not own/rent, but live with another family: The homeowner/lessee must be present at the time of registration. Parent/guardian of the student will still be required to provide proof of residency at the Perry address.
- Child’s immunization records
- Court/custody documentation (if applicable)

PRESCHOOL
STEP 1:
You must first contact the preschool coordinator and set up a screening appointment for your child. Before any preschool student can be enrolled, they must be accepted into the program, as there is limited availability.
Preschool is only open to Perry Local District Residents.
